How to Get a Zepbound Prescription in Ontario (2026)

Zepbound's Health Canada authorization expressly includes chronic weight management, with body mass index (BMI) eligibility criteria written into the monograph — the same on-label shape as Wegovy, and a different shape from Ozempic or Mounjaro, whose own indications are diabetes-only, off-label for weight management. That authorized shape doesn't change at the provincial border: an Ontario prescriber assesses the same BMI-based criteria as anywhere else in Canada. What does change locally, and what this page covers instead: what an Ontario pharmacist can and can't do for you directly, which telehealth providers have confirmed they serve Ontario and what Zepbound costs through the one that carries it directly, how the province's public drug programs actually treat a weight-management prescription, and where to fill it once you have one.

Your family doctor. OHIP "covers the full cost of your services — as long as they're medically necessary" (ontario.ca, What OHIP covers), so billing a medically necessary consultation carries no direct charge to you. A doctor who already has your chart is also better positioned to document the measured height, weight, and any weight-related comorbidity the BMI criteria reference, rather than relying on what you can report yourself.

A walk-in clinic. Covered by OHIP on the same basis, if the clinic bills the province directly. A clinician meeting you for the first time can take a measurement in the room, but won't have your prior history of comorbidities or treatments on file — exactly the kind of documentation a private insurer's prior-authorization review tends to ask for later.

A pharmacist. In Ontario specifically, a pharmacist cannot start a new Zepbound prescription. The Ontario College of Pharmacists' independent-prescribing list covers defined minor ailments plus smoking-cessation drugs, oseltamivir for influenza, and nirmatrelvir/ritonavir for COVID-19 — none of which reach a GLP-1-class medication (Ontario College of Pharmacists, published Oct 1, 2024).

Where an Ontario pharmacist can help is with a Zepbound prescription a physician or nurse practitioner already wrote: renewing it for continuity of care, capped at the lesser of the original quantity or a 12-month supply and excluding controlled substances and monitored drugs, and separately adapting its dose, format, regimen, or route — an authority that explicitly excludes therapeutic substitution (Ontario College of Pharmacists).

Alberta is the only jurisdiction in Canada where independent pharmacist prescribing for any Schedule I drug is implemented, and even there it's limited to pharmacists holding a specific added credential — Ontario is neither. That combined pricing line is worth pausing on, not just skimming past: the Zepbound monograph's own Limitations of Use state that "coadministration with other tirzepatide-containing products (e.g., Mounjaro®) ... is not recommended" — so if you're currently taking Mounjaro, say so on your intake. If cost is the deciding factor and Zepbound itself isn't a requirement, it's worth knowing what else is available through the same intake. Apo-semaglutide — Health Canada's generic version of Ozempic — is a different medication, not a substitute for Zepbound, but it's priced at a flat $149/month at the 0.25, 0.5 and 1.0 mg pen strengths through Felix, off-label for weight management. Whether either drug is appropriate for you is a question for a licensed practitioner, not a price comparison. Ontario's public drug program is the Ontario Drug Benefit (ODB) and the Trillium Drug Program — a public, income-based program. Ontario also runs the Exceptional Access Program (EAP), a prescriber-initiated request process for drugs reviewed outside the standard formulary. Those programs exist as processes regardless of which GLP-1-class drug you're asking about.

What differs for Zepbound specifically is the likely outcome, not the process. The CMA notes that coverage "looks very different for weight management" than for diabetes: only a small number of private plans offer partial coverage for GLP-1 drugs approved for chronic weight management, and public drug plans generally do not cover them at all (CMA) — our own national Zepbound guide states the same fact. Not having a family doctor doesn't disqualify you from care, and it's common enough to be the norm for millions of people across Canada, Ontario included. Two routes still work without one: a walk-in clinic (OHIP-billed if the clinic bills the province directly) and telehealth. Whichever you choose, bring what you can — recent bloodwork, blood pressure readings, prior diagnoses, and your pharmacy's medication history — since Zepbound's BMI-and-comorbidity criteria are usually easier to document from an existing chart than from memory alone.

The same fraud warning signs apply in Ontario as anywhere in Canada. The CMA states plainly that if a site says you can get a GLP-1 drug without a prescription, that's a major red flag (CMA). Health Canada adds that a legitimate online pharmacy "Requires a valid prescription from a physician or other health practitioner licensed to practice in Canada," is licensed by a provincial or territorial pharmacy regulatory authority — in Ontario, that's the Ontario College of Pharmacists — and provides a street address located in Canada (Health Canada, Choosing a safe online pharmacy). Because coverage for weight-management drugs is limited, cost pressure is real — and that's exactly what unauthorized sellers target. A price far below what licensed Canadian pharmacies charge is a warning sign, not a bargain.
