Generic Semaglutide and Your HSA in Canada

Eligibility is not what changes when your prescription is filled with a generic. The arithmetic is. A health spending account gives you a fixed number of credits, and the price of the product decides how many months those credits cover.

This page is about that trade-off, and about the one paperwork detail that catches people out when a pharmacy substitutes.

Eligibility does not change with the generic

The tax test an HSA administrator applies has two parts and neither mentions brands. CRA's Income Tax Folio S1-F1-C1 ¶1.123 sets out paragraph 118.2(2)(n): the drug must be one that can "only be lawfully acquired if it is prescribed by a medical practitioner or dentist for use by the patient", and "the purchase of the drug is recorded by a pharmacist" (accessed 30 September 2026).

A generic semaglutide dispensed by a Canadian pharmacy on a prescription satisfies both parts the same way a brand does. What can differ is your own plan's design — some employer plans have their own rules about substitution or about what they will reimburse — so confirm with your administrator rather than assuming the two are interchangeable on a claim form.

Generic semaglutide reaches Canadian pharmacies under more than one name, and the receipt will show the one that was actually dispensed:

Substitution, and what the receipt has to say

The claim is assessed on the pharmacy's record, not on what the prescription said. If your prescription was written for a brand and filled with a generic, the printout will name the generic, and that is the name your claim needs to match.

Check that the receipt shows the product that was dispensed with its DIN, alongside the patient name, strength and quantity, prescriber, dispensing date, the amount you paid and the pharmacy. myHSA's own list for semaglutide prescriptions asks for "the drug's information, including its name (e.g., Ozempic) and DIN numbers if applicable" as well as the patient's name matching the account, the prescriber's name and credentials, and "the total amount paid and the dispensing date" (myHSA, Is Ozempic (Semaglutides) Eligible under an HSA?, accessed 30 September 2026). A mismatch between the product named on the claim and the product on the receipt is an avoidable rejection.

The balance arithmetic

This is the whole reason the generic matters to an HSA holder. Credits are a fixed number of dollars; the monthly price is the divisor. Lower the divisor and the same balance covers more months — which also means fewer months where the unreimbursed remainder lands on your own card.

Two things to do the arithmetic properly. First, compare the same strength and quantity — a per-pen price and a per-month price are not the same number. Second, include the dispensing fee, since that is part of what your credits will be asked to cover.

Ask your drug plan how it prices the generic

Before you work out what your HSA has to cover, ask your drug plan two questions, because the answers change the arithmetic above the HSA line:

• Does the plan reimburse up to the price of the lowest-cost equivalent? If it does, the plan's share is calculated on the generic whether or not you are dispensed the generic — and if you choose the brand, the difference falls to you or to your credits.
• Does the plan require substitution, or allow the brand on request? That is a plan-design question and a clinical one; your prescriber and pharmacist deal with the second part.

Compare pack sizes, not just labels

Two quiet traps when you compare a generic against a brand on a fixed balance:

• Quantity. Pens, pack sizes and the number of weekly doses per pen differ between products and strengths. A per-pack price and a per-month cost are different numbers, and only one of them tells you how far your credits stretch.
• The dispensing fee. It is charged per fill, so a product dispensed in smaller quantities can carry more fees over a year even at a lower unit price. The fee appears on the same prescription receipt and is usually part of what your HSA is asked to cover.

Where the credits fit with everything else

An HSA sits behind your other coverage, so the order still matters even when the product is cheaper:

1. Your drug plan first — start with the coverage checker and the prior-authorisation walkthrough.
2. A manufacturer savings card, if you qualify and if it applies to the product dispensed. Brand programs do not generally apply to a generic; see savings cards.

Is generic semaglutide HSA eligible in Canada?

It meets the same two conditions a brand does — a drug that lawfully requires a prescription, recorded by a pharmacist — under CRA folio S1-F1-C1 ¶1.123. Your own plan design governs, so confirm with your administrator.

Does switching to the generic change my eligibility?

Not the tax test. It changes the amount, and therefore how long your credits last.

Will a brand savings card still apply?
