Can You Use an HSA for Ozempic in Canada?

Short answer: an Ozempic prescription is normally an eligible expense under a Canadian health spending account, because it is a drug that lawfully requires a prescription and is dispensed and recorded by a pharmacist. Those are the two conditions the tax rules turn on, and your administrator applies them. What actually decides whether you get reimbursed is your plan's own design, your balance, and whether your receipt has the right fields on it.

This page is about paying for the prescription β€” eligibility, receipts and price. It says nothing about what the medication does; that conversation belongs with your prescriber or pharmacist.

The rule your plan administrator is applying

A health spending account in Canada is an employer-funded account whose tax status comes from the private health services plan (PHSP) rules. Administrators reimburse what would qualify for the medical expense tax credit, so the drug test is CRA's drug test.

CRA's Income Tax Folio S1-F1-C1 ΒΆ1.123 sets out paragraph 118.2(2)(n): an amount paid for a drug qualifies where the drug "can only be lawfully acquired if it is prescribed by a medical practitioner or dentist for use by the patient", and "the purchase of the drug is recorded by a pharmacist" (accessed 30 September 2026). CRA's RC4065 Medical Expenses states the same requirement and adds that over-the-counter medications, vitamins and supplements do not qualify "even if prescribed by a medical practitioner" (accessed 30 September 2026).

Ozempic is a prescription-only product dispensed through a pharmacy, so it lands on the right side of both conditions. myHSA, a Canadian HSA administrator, publishes the same conclusion for semaglutide products in its help centre and states that the medication's indication is not part of the test, provided three things hold: the prescription came from a qualified health professional, the drug was dispensed by a licensed pharmacy rather than an unauthorised online source, and the receipt carries the required details (myHSA, Is Ozempic (Semaglutides) Eligible under an HSA?, accessed 29 September 2026).

That is the general rule. It is not a promise about your plan: an employer's plan design can be narrower than CRA's list, and only your administrator can confirm what yours pays.

Three things that get an Ozempic claim denied

1. The wrong kind of receipt. A debit or credit slip proves you paid something; it does not prove what, for whom, or on whose prescription. CRA's documentation standard for medical expenses is a record showing "the purpose of the payment, the date of payment, the name of the patient, and, if applicable, the medical practitioner who prescribed the purchase or gave the service" (CRA, RC4065 Medical Expenses, accessed 30 September 2026). 2. Product bought outside a licensed pharmacy. Compounded or unauthorised product bought online is not a pharmacist-recorded dispense of a prescription drug, so it fails the second half of the CRA test. Health Canada is direct about where to buy: "In Canada, only licensed pharmacies can legally sell prescription drugs", and unauthorised or counterfeit products "have not been assessed by Health Canada for safety, effectiveness, or quality" (Health Canada, Thinking about buying GLP-1 drugs like Ozempic or Mounjaro?, 21 January 2026, accessed 30 September 2026). Buy through a licensed Canadian pharmacy.

3. Double-claiming. If your drug plan already reimbursed part of the cost, only the remaining balance is claimable. CRA's rule for the tax credit is that you can claim only the part of an expense "for which you have not been and will not be reimbursed" (CRA, lines 33099 and 33199, accessed 30 September 2026), and plans coordinate on the same basis.

The receipt, field by field

Ask the pharmacy for an official prescription receipt β€” not the till slip. Most Canadian pharmacies will print a prescription history for any date range on request, which is the easiest way to clear a year of claims in one submission.

Submit under your plan's medicines or prescription-drug category, and keep the receipt afterwards β€” CRA tells taxpayers not to send documents with a return but to keep them "in case the Canada Revenue Agency (CRA) asks to see them later".

Compare the price before you spend the credits

HSA credits are finite and, in most plans, expiring. What Ozempic costs at the counter is the one variable you can move without asking anyone's permission, and the spread between Canadian pharmacies on the same strength and quantity is not small.

Generic semaglutide and your balance

Generic semaglutide is now available in Canada, and the eligibility test does not change with the product: it is still a prescription drug recorded by a pharmacist. What changes is how far a fixed HSA balance goes.

If your prescription is dispensed as a generic, check that the receipt names the product actually dispensed. The claim is assessed on what the pharmacy recorded, not on what the prescription said.

If your HSA will not cover the whole cost

Work through the payers in this order, because it leaves the most HSA credit available:

  1. Your drug plan. Start with the coverage checker and the prior-authorisation walkthrough.

FAQ

Is Ozempic an eligible HSA expense in Canada?

Normally yes. It is a prescription-only drug dispensed and recorded by a pharmacist, which are the two conditions in paragraph 118.2(2)(n) as described in CRA folio S1-F1-C1 ΒΆ1.123. Your own plan design governs, so confirm with your administrator.

Does my plan need to know why it was prescribed?

The CRA drug test does not turn on the indication β€” it turns on legal prescription status and the pharmacist record. myHSA states the same position. Some employer plans ask for more; ask yours.

What does my pharmacy receipt have to show?

Patient name, drug name and DIN, strength and quantity, prescriber, dispensing date, amount paid and the pharmacy. A debit slip or till receipt on its own is not enough.

Can I use my HSA if my drug plan already paid part of the cost?

Yes β€” for the unpaid balance. You cannot claim a portion that was already reimbursed.

Can I claim an HSA-reimbursed prescription on my taxes?

No. The medical expense tax credit applies only to amounts you were not and will not be reimbursed for, unless the reimbursement was included in your income.

Does it matter which pharmacy I use?
