Are Telehealth Fees HSA Eligible in Canada?

A prescription bought through a virtual-care service usually arrives as more than one charge: a consultation or assessment fee, sometimes a membership or subscription fee, the medication itself, a dispensing fee, and possibly shipping. A health spending account does not necessarily handle those charges the same way, and the split does not follow the invoice — it follows the tax rules the administrator is applying.

Not tax or medical advice. Plan designs differ, and so do provincial rules about who counts as an authorised practitioner. Confirm with your plan administrator before you submit, and with the Canada Revenue Agency or your own adviser on tax questions.

The test is who provided the service, and where

For a service, eligibility turns on whether the person who provided it is an authorised medical practitioner — and that is a provincial question, not a national one. CRA publishes a table, *Authorized medical practitioners for the purposes of the medical expense tax credit*, listing professions row by row with a column for each province and territory (accessed 30 September 2026). A profession authorised in one jurisdiction may not be listed in another, so the same virtual consultation can land differently depending on where the service was provided.

For a drug, the test is the one described in Income Tax Folio S1-F1-C1 ¶1.123: the drug must be one that can "only be lawfully acquired if it is prescribed by a medical practitioner or dentist for use by the patient", and "the purchase of the drug is recorded by a pharmacist" (accessed 30 September 2026).

Those are two different tests applied to two different lines on the same bill.

Line by line

A consultation or assessment fee charged by a physician or nurse practitioner. This is the strongest case: a fee paid to an authorised practitioner for a medical service. Whether the visit happened by video, phone or in person is not what the rule turns on. Check the practitioner's profession against CRA's table for your province.

A membership, subscription or platform fee. Weakest case. A fee that buys access to a service — rather than paying an authorised practitioner for a medical service provided to you — is not obviously a medical expense, and administrators commonly decline it. If your invoice bundles a subscription with a consultation, ask the provider for an itemised receipt that separates them, because an unsplit "monthly membership" line gives your administrator nothing to assess.

The medication. Eligible on the drug test above, provided it was dispensed and recorded by a pharmacist. Health Canada is direct on where that has to happen: "In Canada, only licensed pharmacies can legally sell prescription drugs", and unauthorised or counterfeit products "have not been assessed by Health Canada for safety, effectiveness, or quality" (Health Canada, Thinking about buying GLP-1 drugs like Ozempic or Mounjaro?, 21 January 2026, accessed 30 September 2026).

The dispensing fee. It is part of what the pharmacy charged you and it appears on the prescription receipt, which is why HSAs are routinely asked to cover it when a drug plan caps it. CRA's published wording for drugs describes the amount paid for a drug recorded by a pharmacist and does not separately address the fee, so confirm how your plan handles it.

Shipping and delivery. Usually the weakest line on the invoice after a subscription fee. It is a delivery charge, not a medical service or a drug. Expect to pay it yourself unless your plan says otherwise.

Why a flat monthly telehealth price is awkward on a claim form

Some Canadian providers quote one all-in monthly figure that covers the prescription, the prescriber's involvement and delivery. That is easy to compare against a pharmacy price and hard to submit to an HSA, because the administrator is assessing a drug line and a service line separately and the invoice presents neither.

If you are paying a bundled price, ask the provider for an itemised statement showing the medication (with its name, strength, quantity and DIN), the pharmacy that dispensed it, the amount attributable to the medication, and any consultation fee separately. Renewals and repeat fills

If a line is declined

A declined line is not always a final answer, and the reason matters. Ask the administrator which test it failed: the practitioner (the profession is not authorised in your province), the documentation (the invoice does not show who provided what), or the plan's own design (your employer's list is narrower than CRA's). The first two are sometimes fixable with a corrected, itemised receipt from the provider. The third is not, and it is worth knowing before the next bill.

Before you spend credits on a bundled price

Compare what the medication alone costs at a pharmacy before you decide the bundle is the cheaper route, because only part of a bundle is likely to be reimbursable.
