Ozempic coverage in Ontario

Public plan: Ontario Drug Benefit (ODB)

In Ontario, the public plan is the Ontario Drug Benefit (ODB) program. It covers people 65 and older, people 24 and under who have no private plan (through OHIP+), and people who live in long-term care, receive home and community care, receive Ontario Works or Ontario Disability Support Program benefits, or are enrolled in the Trillium Drug Program.

On the ODB Formulary, Ozempic and generic semaglutide are Limited Use (LU) products. ODB pays for an LU product only when the plan's published LU criteria are met and the prescriber gives the pharmacy the product's Reason for Use code. Each pen has its own code: 665 for the 4 mg pen (1.34 mg/mL, 3 mL), 666 for the 2 mg pen (1.34 mg/mL, 1.5 mL) and 667 for the 2 mg pen (0.68 mg/mL, 3 mL). The criteria for each code are on its page in the Ontario e-Formulary, and the LU authorization period is indefinite.

Generic semaglutide from Apotex and Dr. Reddy's was listed on the formulary on June 30, 2026. ODB now pays brand Ozempic only up to the generic price; the ministry pays more for a brand product only in the medically necessary "No Substitution" cases the formulary describes.

Wegovy appears in the e-Formulary only as an off-formulary interchangeable product, which the ministry does not pay for.
