Saskatchewan Blue Cross has told members that coverage rules for semaglutide products, including Ozempic and its newly approved generic versions, are not changing as a result of Health Canada's authorization [Source: sk.bluecross.ca/advice-centre/blog/generic-ozempic-approved-in-canada-what-you-need-to-know/].

What the insurer is telling members

In a member advisory, Saskatchewan Blue Cross stated that if semaglutide is not covered under a member's plan today, that will not change now that a generic is available [Source: sk.bluecross.ca/advice-centre/blog/generic-ozempic-approved-in-canada-what-you-need-to-know/]. Plans that do cover semaglutide will continue to require members to meet Special Authorization requirements and receive approval before reimbursement [Source: sk.bluecross.ca/advice-centre/blog/generic-ozempic-approved-in-canada-what-you-need-to-know/].

The insurer noted that members approved for coverage may see a generic version cost less than the brand-name product, which could lower out-of-pocket amounts such as co-insurance and reduce pressure on annual dollar limits [Source: sk.bluecross.ca/advice-centre/blog/generic-ozempic-approved-in-canada-what-you-need-to-know/].

Where generic substitution matters most

According to the advisory, the biggest impact will be felt under plans with generic substitution or mandatory generic substitution rules, where claims are reimbursed up to the cost of the generic drug [Source: sk.bluecross.ca/advice-centre/blog/generic-ozempic-approved-in-canada-what-you-need-to-know/]. Saskatchewan Blue Cross also cautioned that new generic medications may take time to become widely available and that members may not see changes at the pharmacy right away [Source: sk.bluecross.ca/advice-centre/blog/generic-ozempic-approved-in-canada-what-you-need-to-know/].

The insurer pointed members to its service line at 1.800.667.6853 for additional questions [Source: sk.bluecross.ca/advice-centre/blog/generic-ozempic-approved-in-canada-what-you-need-to-know/]. Members can also check our insurance coverage checker for plan-by-plan details.

The regulatory backdrop

Health Canada authorized the first generic semaglutide injection on April 28, 2026, making Canada the first G7 country to approve a generic version of the molecule [Source: canada.ca/en/health-canada/news/2026/04/canada-becomes-the-first-g7-country-to-approve-a-generic-version-of-semaglutide.html]. The submission was filed by Dr. Reddy's Laboratories and is authorized for the once-weekly treatment of adult patients with type 2 diabetes to manage blood sugar levels [Source: canada.ca/en/health-canada/news/2026/04/canada-becomes-the-first-g7-country-to-approve-a-generic-version-of-semaglutide.html].

Health Canada is currently reviewing eight other generic semaglutide submissions from different companies and expects to make additional regulatory decisions in the coming weeks and months [Source: canada.ca/en/health-canada/news/2026/04/canada-becomes-the-first-g7-country-to-approve-a-generic-version-of-semaglutide.html]. Our generic semaglutide tracker follows these decisions as they are published.

Coverage gap remains a barrier

Obesity Canada Scientific Director Dr. Sanjeev Sockalingam has highlighted that fewer than 20 per cent of Canadians with private drug benefit plans have access to Health Canada-approved obesity treatment medications [Source: obesitycanada.ca/news/health-canada-generic-semaglutide-approval/]. He has urged governments, insurers, and employers to revisit how obesity care is covered as lower-cost approved options become available [Source: obesitycanada.ca/news/health-canada-generic-semaglutide-approval/].

The Saskatchewan Blue Cross advisory illustrates a key point Obesity Canada has raised: regulatory approval of a generic does not automatically translate into expanded plan coverage. Members whose plans exclude semaglutide today will continue to face the same exclusion after generic launch [Source: sk.bluecross.ca/advice-centre/blog/generic-ozempic-approved-in-canada-what-you-need-to-know/].

What to watch next

  • Additional approvals: Health Canada has eight more generic semaglutide submissions under review [Source: canada.ca/en/health-canada/news/2026/04/canada-becomes-the-first-g7-country-to-approve-a-generic-version-of-semaglutide.html].
  • Pharmacy availability: Saskatchewan Blue Cross notes that generics may take time to become widely available at retail [Source: sk.bluecross.ca/advice-centre/blog/generic-ozempic-approved-in-canada-what-you-need-to-know/].
  • Plan reviews: Whether other insurers follow Saskatchewan Blue Cross's stance of leaving coverage rules unchanged.
