Canadian Benefits Plans Face GLP-1 Strategy Reckoning in 2026

Canadian employer benefits plans are being forced to rewrite their GLP-1 strategies in 2026 as generic semaglutide enters the market, manufacturer savings cards expand, and tirzepatide products remain on a separate, decade-long patent timeline.

Public Plan Spending Pressure

According to benefits advisory firm Sterling Brokers, Ozempic alone accounts for roughly $807 million of public drug plan spending in Canada and was responsible for 8.5 of the 9.2 percentage points of one recent annual increase in public drug spend. On the private side, GLP-1 products remain a top driver of employer drug-plan cost growth.

Generic Semaglutide Changes the Math

Health Canada authorized its first generic version of Ozempic from Dr. Reddy's Laboratories on April 28, 2026, followed days later by a second generic from Apotex, and has since authorized Apotex's Sevmia as the first generic version of Wegovy. Roughly seven to nine additional generic submissions remain under Health Canada review.

Generics in Canada typically run 45% to 90% cheaper than brand, with near-term reductions of 30% to 50% viewed as realistic for plan sponsors through mid-to-late 2026. Readers tracking authorizations can follow our generic semaglutide tracker.

Novo Nordisk Savings Card Expansion

On July 6, 2026, Novo Nordisk Canada expanded its Ozempic Savings Card to eligible Canadians with private drug coverage, not just those paying entirely out of pocket. Under the expanded program, plan members can buy brand-name semaglutide at a price the company says is aligned with generic alternatives, with the discount applied automatically at pharmacies nationwide (Quebec excluded) through Novo Nordisk Care Rx, operated by Rexall, and select telehealth and retail partners.

Sterling Brokers notes that manufacturer savings cards can influence which product gets dispensed, which may work against a plan's generic-substitution and cost-containment strategy. Members comparing coverage options can use our insurance coverage checker.

Tirzepatide Products on a Different Timeline

The tirzepatide-based Mounjaro and Zepbound brands are not following semaglutide's generic trajectory. Eli Lilly holds patent protection in the United States until 2036, and a true generic tirzepatide is unlikely for another decade.

Eli Lilly did reduce Canadian list prices for both Mounjaro and Zepbound effective December 29, 2025, with the lowest-dose KwikPen tiers listing at approximately $300 per 4-week supply and the highest tiers at approximately $540. Current verified pharmacy prices on GLP1Prices.ca show Mounjaro ranging from $245 to $800 CAD and Zepbound from $276.66 to $609 CAD depending on dose, pharmacy, and province.

Health Canada Warns on Unauthorized Products

As demand grows, Health Canada published advisory RA-81518 on January 21, 2026, warning Canadians about unauthorized and counterfeit versions of GLP-1 receptor agonist products, and advising consumers to buy prescription drugs only from licensed pharmacies. The agency said it has identified various retailers in Canada selling unauthorized GLP-1 products, both in stores and online, with some websites misusing official Health Canada logos and displaying fake endorsements.

Health Canada advised consumers to verify authorization by checking for the eight-digit Drug Identification Number on the label and searching the Drug Product Database, and said it is working with the Canada Border Services Agency to stop unauthorized shipments.

Employer Posture Shifting

Sterling Brokers reports that most Canadian employer plans now offer some GLP-1 coverage, but tightly managed, with prior authorization, BMI thresholds, step therapy, and annual or lifetime maximums common where coverage exists. The firm notes that 72% of employers cite high discontinuation and regain as reasons not to cover GLP-1 products, and among large employers the posture has shifted from cautious expansion to cautious contraction.
