Plan Sponsors Brace for 65% Semaglutide Price Cut in Canada

Canadian private drug plans are preparing for a mandated 65 percent list-price reduction on semaglutide once three generic manufacturers reach the market, a shift that follows the January 4 lapse of the molecule's Canadian patent protection.

What Changes for Plan Sponsors

Bloomberg's reporting, summarized by Benefits and Pensions Monitor, confirms that the core patent for semaglutide β€” the molecule in Ozempic and Wegovy β€” has already lapsed in Canada after missed maintenance fee payments and the expiry of data exclusivity on January 4. Once three generic competitors are on the market, Canadian policy will drive list-price discounts of about 65 percent.

That mandated reduction in a high-volume chronic therapy has direct implications for private plans that currently see GLP-1 claims largely paid out of pocket in many countries, according to analysts cited by Bloomberg.

The Pipeline Behind the Price Cut

Health Canada currently has nine generic semaglutide applications under review and is working through a broader backlog of drug files. Sandoz, which had aimed to launch in the first half of 2026, now expects the Canadian generic market to take shape in the third quarter of 2026, CEO Richard Saynor said. The delay came after both Sandoz and Reddy's Laboratories were told they must provide more data to meet regulatory standards.

Aspen Pharmacare plans to launch its generic semaglutide first in Canada and emerging markets before moving into Mexico, New Zealand, Europe and the US, which positions Canada as an early test of pricing, access and spillover into other systems.

GLP1Prices.ca's generic semaglutide tracker shows Health Canada has already approved several generic products, including Sevmia from Apotex on June 29, 2026, which references Wegovy.

Scale of the Canadian Market

Sandoz describes Canada as the world's second-largest semaglutide market. Karine Matteau, vice-president in charge of the commercial office of Sandoz Canada, told The Logic that "Canada is the second-largest semaglutide market in the world" after the United States.

The pan-Canadian Pharmaceutical Alliance's tiered framework for injectables and other non-oral-solid forms sets the price at 35 percent of the brand reference price once three or more competing manufacturers are counted in the relevant category. The framework also considers market status and supply, not just approval counts.

Where Brand Prices Stand Today

Current pharmacy listings on GLP1Prices.ca show brand Ozempic ranging CAD $86.40 to $663, Wegovy ranging $270 to $550, and Rybelsus ranging $230 to $293.99. Eli Lilly's Mounjaro and Zepbound are not affected by the semaglutide patent lapse; Mounjaro still has about a decade of patent life.

Novo Nordisk's Response

Bloomberg reports that Novo has responded by changing its CEO, cutting prices, launching a Wegovy pill and advancing new products such as CagriSema and amycretin. It has also partnered with local companies to sell semaglutide under a second brand in India and Brazil, and reduced Wegovy prices in India and China.

IQVIA warned that as more manufacturers enter, "the dangers of counterfeiting and substandard quality are likely to be multiplied," as cited by Bloomberg.
