Generic Semaglutide Hits Tier 2 Pricing: ~$114 Per 4-Week Supply

Because Health Canada authorized generic semaglutide products from Dr. Reddy's Laboratories and Apotex within three days of each other, the Canadian market has bypassed Tier 1 of the pan-Canadian Pharmaceutical Alliance generic pricing framework and moved directly to Tier 2, capping the maximum public-plan price at approximately $114 per four-week supply [Source: walkin.ca/blog/generic-ozempic-canada/].

How the back-to-back approvals reshaped pricing

Health Canada authorized the first generic semaglutide injection, filed by Dr. Reddy's Laboratories, on April 28, 2026, making Canada the first G7 country to approve a generic version of the drug [Source: canada.ca/en/health-canada/news/2026/04/canada-becomes-the-first-g7-country-to-approve-a-generic-version-of-semaglutide.html]. The pCPA tier math

Under the pan-Canadian Pharmaceutical Alliance framework for injectable generics, prices step down as more manufacturers enter the market [Source: walkin.ca/blog/generic-ozempic-canada/]:

  • Tier 1 (one generic manufacturer): price capped at 75% of brand, eventually dropping to 55% [Source: walkin.ca/blog/generic-ozempic-canada/]
  • Tier 2 (two generic manufacturers): price drops to 50% of brand reference price [Source: walkin.ca/blog/generic-ozempic-canada/]
  • Tier 3 (three or more generic manufacturers): price drops to about 35% of brand reference price [Source: walkin.ca/blog/generic-ozempic-canada/]

CBC News reported a similar tier structure, noting that the first generic could cost 75 to 85 per cent of the brand name, that the price for both drops to 50 per cent once a second medication enters the market, and that the cost would decrease to about 35 per cent of brand-name Ozempic's price when three or more generic products are available [Source: cbc.ca/news/health/ozempic-semaglutide-generic-version-canada-9.7184915].

The current Ontario Drug Benefit formulary list price for a four-week supply of branded Ozempic is $227.94, before pharmacy markups and dispensing fees [Source: walkin.ca/blog/generic-ozempic-canada/]. Applying the Tier 2 50% reduction produces the approximately $114 maximum public-plan price now in effect [Source: walkin.ca/blog/generic-ozempic-canada/].

Retail cash prices remain higher

The pCPA-mandated price applies to public drug plans. For patients paying out of pocket without private insurance, the actual retail cost at the pharmacy counter typically falls between $300 and $450 per month [Source: walkin.ca/blog/generic-ozempic-canada/]. CBC reported that, depending on the dose, people can pay about $300 to $400, sometimes even more, per month for brand-name Ozempic [Source: cbc.ca/news/health/ozempic-semaglutide-generic-version-canada-9.7184915]. Catherine Thomas, Apotex's vice president of global communications, told CBC News that the company's product should be available to Canadians within weeks and that for most private plans the process to include generics happens shortly after launch [Source: cbc.ca/news/health/ozempic-semaglutide-generic-version-canada-9.7184915].

More approvals could push prices to Tier 3

Health Canada is currently reviewing seven other submissions for generic semaglutide by different companies and expects to make regulatory decisions on more of these submissions in the coming weeks and months [Source: canada.ca/en/health-canada/news/2026/05/canada-approves-second-generic-semaglutide-the-first-g7-country-to-do-so.html].
