Plan Sponsors Brace for Canada's 65% Semaglutide Price Trigger

Canadian private drug plans are preparing for a mandated 65% list-price reduction on semaglutide once a third generic competitor enters the market, a threshold Novo Nordisk itself has acknowledged as a defining moment for the Canadian GLP-1 category.

How Canada got here first

Health Canada authorized the first generic semaglutide injection on April 28, 2026, a submission filed by Dr. Reddy's Laboratories, making Canada the first G7 country to clear a generic version of the molecule. A second generic from Canadian manufacturer Apotex followed shortly after, and Health Canada is currently reviewing seven other generic Ozempic submissions. The regulatory door opened after the core patent lapsed in Canada following missed maintenance fee payments and the expiry of data exclusivity on January 4, 2026. BMJ reported the approval was made possible after Novo Nordisk did not pay a C$250 annual fee.

The 65% trigger

Emil Kongshøj Larsen, Novo Nordisk's executive vice president of international operations, told analysts on the company's most recent earnings call that once three generic competitors are on the Canadian market, national policy mandates a 65% cut to Novo's list price. Larsen said Novo sees a low single-digit impact from the generics and that its "leading tactic" in Canada is the savings card for both Ozempic and Wegovy. Sandoz pushes timing to Q3

Sandoz CEO Richard Saynor said the Canadian generic market is now expected to take shape in the third quarter of 2026, after the company had originally aimed to launch in the first half of the year. The delay followed a request from Health Canada that both Sandoz and Dr. Reddy's provide additional data to meet regulatory standards. Sandoz has described Canada as the world's second-largest semaglutide market.

Aspen chooses Canada first

Aspen Pharmacare plans to launch its generic semaglutide first in Canada and emerging markets before moving into Mexico, New Zealand, Europe and the United States. That launch sequence positions Canada as an early test of pricing, access and spillover into other systems.
