Tirzepatide Stays Brand-Priced in Canada as Semaglutide Generics Arrive

Canadian benefits advisors are warning plan sponsors that the two tirzepatide brands sold in Canada, Mounjaro and Zepbound, will remain brand-priced for years even as generic semaglutide reshapes the rest of the GLP-1 category, because Eli Lilly holds U.S. patent protection on tirzepatide until 2036.

Two GLP-1 Timelines Diverge

Sterling Brokers, in an advisory note to Canadian employers, said Health Canada authorized its first generic version of Ozempic (Dr. Reddy's Laboratories) on April 28, 2026, followed days later by a second from Apotex, and has since approved the first generic version of Wegovy for weight management under Apotex's Sevmia brand. Roughly seven to nine additional generic semaglutide submissions remain under Health Canada review.

The firm noted that while the U.S. FDA has agreed to review generic tirzepatide applications from Sandoz, Eli Lilly's U.S. patent runs to 2036, and a true tirzepatide generic is unlikely for another decade.

Where Tirzepatide Pricing Sits Today

Eli Lilly filed updated Canadian list prices for Mounjaro and Zepbound effective December 29, 2025, ahead of new Patented Medicine Prices Review Board guidelines that took effect January 1, 2026. The revised list prices, per 4-week supply, are approximately $300 for the 2.5 mg and 5 mg doses, $420 for 7.5 mg and 10 mg, and $540 for 12.5 mg and 15 mg.

Verified Canadian pharmacy prices tracked on GLP1Prices.ca currently range from $245 to $800 for Mounjaro and $276.66 to $640 for Zepbound, reflecting dispensing fees and dose tier.

Only KwikPen in Canada

Zepbound was approved by Health Canada on May 13, 2025, and Eli Lilly launched the brand at Canadian pharmacies on July 9, 2025. In Canada, Zepbound is sold exclusively as the KwikPen, a multi-dose prefilled pen containing four weekly doses, a different format from the single-dose auto-injector pens Zepbound launched with in the United States.

Mounjaro and Zepbound contain the same active ingredient, tirzepatide, at the same doses and delivered in similar pens, but Lilly markets them as separate brands so each can carry its own Health Canada-authorized label and packaging. Mounjaro has been available in Canada under the Mounjaro brand since 2022.

What It Means for Plan Sponsors

Sterling Brokers argued that as prescribing patterns shift toward tirzepatide, Canadian plans could see utilization migrate toward drugs that will stay brand-priced for another decade, partially offsetting savings from cheaper semaglutide generics. The firm advised that coverage design should account for this substitution effect rather than today's drug mix alone.

Generics in Canada typically run 45% to 90% cheaper than brand, and analysts expect plan sponsors to begin seeing meaningful savings on semaglutide through mid-to-late 2026, with near-term reductions of 30% to 50% viewed as realistic. No comparable trajectory exists for tirzepatide on the Canadian market.

Key developments Canadian plan sponsors are tracking:

  • Semaglutide generics: First two approved by Health Canada in late April 2026, with up to nine more under review.
  • Tirzepatide brand pricing: Lilly's revised Canadian list prices range from about $300 to $540 per 4-week supply across doses.
  • Format: Zepbound is sold in Canada only as the KwikPen.
  • Patent horizon: U.S. tirzepatide patent protection extends to 2036.
