Generic semaglutide pricing in Canada will step down according to the pan-Canadian Pharmaceutical Alliance tiered framework, with the steepest reductions arriving only after multiple manufacturers reach pharmacy shelves.
How the tiered framework works
Under the pan-Canadian Pharmaceutical Alliance generic drug framework, the first authorized generic enters the market priced at a set percentage of the brand-name reference product, with further step-downs as additional manufacturers are authorized and begin shipping. Health Canada has stated that many generic medications in Canada are 45% to 90% cheaper than brand-name counterparts.
Coverage adjustments by private insurers and provincial drug plans typically lag the price change by weeks to months, according to patient-facing guidance published after the first authorization.
Three generics is the inflection point
Mina Tadrous, an associate professor at the University of Toronto who researches drug policy, has said that two more generics beyond the first authorization need to be approved to drive large cost reductions, and that with three generics on the market monthly prices could drop to about $100 or less, depending on dose.
Health Canada confirmed that at the time of the first authorization it was reviewing eight other submissions from different companies, with decisions expected in the coming weeks and months.
Current branded pricing on GLP1Prices.ca
CBC reported that a monthly supply of Ozempic or Wegovy costs between $300 and $400, sometimes more depending on dose. Current pharmacy data on GLP1Prices.ca shows a wider spread: Ozempic listings range from $86.40 to $663 CAD across 85 samples, while Wegovy ranges from $270 to $550 CAD across 93 samples. Readers tracking tirzepatide products such as Mounjaro and Zepbound should note those products are not affected by the semaglutide generic pathway.

Scope of the first authorization
Health Canada authorized the Dr. Reddy's Laboratories generic semaglutide injection for the once-weekly care of adult patients, matching the authorization of the branded reference product. Patient guidance published after the authorization notes that the April 28 generic authorization covers the same authorized use as the reference brand, and does not extend to the separate Health Canada authorization held by Novo Nordisk's chronic weight management brand, which carries distinct labelling and clinical-data requirements.
Health Canada's review found that differences between generic products and the reference biologic do not affect safety, efficacy or quality of the drug.
What patients should expect at the counter
- Automatic substitution is standard across most private plans once a generic is formulary-listed, according to patient guidance.
- No new prescription is typically needed in most Canadian provinces; a pharmacist can substitute unless the prescriber has written "no substitution" or "dispense as written".
- Pharmacy stocking will roll out region by region rather than all at once, with urban pharmacies likely to stock earlier than rural locations.

Timing uncertainty remains
CBC reported that it is unclear when Dr. Reddy's generic will reach the Canadian market, and that the company had not responded to a request for comment at the time of publication. The pace at which the PCPA framework delivers its deepest reductions therefore depends on both regulatory decisions on the pending submissions and the physical arrival of each product in provincial wholesale channels.
